New Patients
Total Foot & Ankle Center, Lake Nona is currently accepting new patients, with many types of insurance. Walk-in appointments are welcome! We request that you provide at least 24-hours notice if you must cancel or reschedule your appointment. Please arrive 15 minutes before your scheduled appointment in order to complete and process the appropriate paperwork.
Please bring your insurance card to every visit. On your first visit, please be sure to bring your driver's license or other form of government-issued identification, names of your other current doctors, list of any medications you are taking, and any x-rays, MRIs, or results of diagnostic tests you have recently undergone.
If you wish to save time before your first visit, you may click the links below to print and complete the five required new patient forms. Be sure to bring them with you to your first evaluation and hand them to the receptionist upon your arrival.

To complete a form Offline, click on one of the Offline links below.
A browser window will open displaying the form. Print the displayed form, fill them out, and bring them to your first appointment. Completing the New Patient Information Forms will save you time on your initial visit!
